Auto Exception Set at Session End

I have seen an issue whereby although i have multiple attempts set on the queue if there is a server disconnect or BP crashes the items are not being added back into the queue.
If the session goes down a termination route and the session ends then anything locked will be added back into the queue and marked as Auto Exception Set at Clean Up.
The issue is that there is nothing i can do in a process to account for server crashes and this then leads to a big risk of work not being completed if the items are not being added back into the queue.

Does anyone know why this would be happening or is it a bug in the software? 6.10.2

Within the configuration for each queue, you should be able to specify a max number of reattempts. If this scenario occurs during the first attempt of a queue item, it should be able to be picked back up on the next go around, assuming the deferral time automatically generated has passed.
