1. If by "manually set the flag for HTML", you mean supplying an argument for the advanced or expert variant of "Get Received Items", then yes - that is how it comes OOB. It's faily easy to make some modifications to support it otherwise. I'm attaching my version of the VBO after I merged it with my original custom mail handler and meddled around with some of the logic. Since the current forum (ironically) does not support .bprelease as an uploadable file type, I've renamed it to .txt . You'll need to change it back to .bprelease before importing.
2. I have not seen this, but the closest scenario that comes to mind is not having programmatic access set up in Outlook.
