Items with duplicated exception

Hello, I'm having some trouble marking an item as an exception, it is always duplicated. 

As I show in the screenshots to a simple process, the Work Step throws the "Example" Exception that is caught by the Main Page of the process, calculates the detail to Mark the Exception Reason, but when I look at my Queue it is duplicated, even if in the Exception Detail is normal. I've realized it doesn't happen with BP Standart Exceptions, and it doesn't happen with any of my crewmates. Even though we use the same initial template. 

Anyone going through the same?

You can analyze by Template V2, even if you don't use it, I highly recommend using it

Thank you guys for the reply!

But after a few more tries I found out it was because I put a space at the end of the exception, when I took it off, it stopped duplicating.
