Document form definition not showing up

Hello, I have a issue with my Decipher 1.2. I wanted to create a new document type but "Document Form Definition" is not showing up and Save button is not working. It is just this black screen

OK, Found the issue. I had to forcefully reload the page by clicking CRTL SHIFT R
