Effort Calculation Document

Does anyone know of a good effort document template? I am thinking of something where you would list out all the points of a build and based on the system, element types, and complexity of the process it would assist in calculating how long it would take to complete the build.   Thank you for any support.

I'm also looking for such Document. But, don't know how & where to start. A little bit help is necessary.

James - I'd be very interested to know if you were able to find a tool which allowed you to calculate effort? I have been looking for something similar for a small amount of time now, and am reluctant to build something from scratch, specifically as I imagine it'd have to be based on guess work initially and change over time.
