The root cause would be that Blue Prism crashed, which causes that popup to appear. As far as the root cause of Blue Prism crashing, is there a specific stage at which it crashes, does it happen every time, what was it doing when it crashed?

Please raise a ticket with Support and provide them with as much information as possible. In addition to Nicholas' advice, the Windows event logs on the resource and application server may reveal clues.
