By manually you mean, running the interactive client, navigating to control room, selecting a work queue, finding the work queue item, right clicking and selecting "Mark selected case and exception" e.g.
and also when marking an item as exception from within a running process?
If so, this looks like a permissions issue; perhaps a strange combination of permissions within the Blue Prism internal roles or a database level permission. I would highly recommend raising a support ticket for this and attaching all the relevant information to the ticket, to do so click here
